You may download and complete a records request form (English) or (Spanish) which can be faxed to 303-441-4330 or mailed to:

To request a background check on an individual, you may download and complete a background check request form (English) or (Spanish).

Dispatch Records:

Requests for copies of 911 tapes from dispatch are handled by the Communication Section. Please complete a Computer-Aided Dispatch request form and fax it to Karen Luebke at 720-564-0335. If you need to speak to Karen, please call her at 303-441-4309.
