H1N1 Request to Resolve Case by Mail

If you cannot come to court due to H1N1 flu and your ticket is not a mail-in ticket, you may use this form to request that your case be resolved without a court appearance. The prosecutor and the judge will review your request to determine if that is possible.You will be notified by email or regular mail of the decision.  Complete this form and submit it to the court before the arraignment date on your ticket.

You will receive written confirmation of the outcome of this requestfrom the court. Indicate below whether you wish to receive notice of your court date by fax or by mail. If you have not received notice of your court date after 5 business days, call the court at (303) 441-1810.
