HSA Bank Account

Open your HSA Optum Health Bank

In order to receive the City contributions to your $1500 Deductible plan Health Savings Account (HSA), you need to set up this bank account with Optum Health Bank no later than November 30, 2012.

The bank application process takes approximately 5 minutes.  Follow this link to Optum to begin.

  • You will need only your driver’s license or passport (do not enter dashes). 
  • City of Boulder plan information will already be completed for you. 
  • Be sure to use your full legal name with your Social Security number, not a nickname. 
  • The “HDHP Effective Date” is 01/01/2013. 
  • Setting up an “initial contribution” in Step 6 will not come through payroll.  If you want your contributions to your HSA to be pretax and come through payroll deduction, please do that through open enrollment with the City, not in this bank application process.

All City of Boulder employees will automatically default into the eAccess account type.  Review the other account types that will be available to you here.  (HSA Account Types )

Once your account is established (can take 2-4 business days after application), a Welcome Kit will be mailed to your home providing all of the details on your new savings account. (Note: you will have the choice during account set up to have this kit emailed to you instead.)  You should also review the HSA FAQs on the “Training” tab of this site for more information on this account.

Two plain white envelopes, with a Salt Lake City, UT mailing address, will arrive at your home within 2 weeks of your approved application.  One envelope contains your HSA debit card and the other your PIN.
