  1. Download and print the appropriate permit application form below or call 303-441-1880 to have one faxed to you.
  2. Complete the application form. 
  3. Fax the application to 303-441-4241 or drop off your application at 1739 Broadway. 
SKIP-A-TRIP fax applications will be processed within two business days, excluding holidays and weekends. 
If the application is insufficient, the areas where information is missing will be circled and the application will be faxed back to you requesting the missing information.

Boulder Construction Use Tax Information
For building permits that have a final cost or contract price of $20,000 or more, project reconciliation must be completed with the City of Boulder Finance Department. These projects may be audited. The Project Cost Reconciliation Return forms and instructions are available at www.bouldercolorado.gov/constructionusetax. Please refer to the "Forms & Instructions" tabs on the left menu and direct any questions about use tax to the City of Boulder Finance Department at 303-441-3050.
