 Waiver of Personal Service and Request for Arraignment (10.82 KB): Use this form to request a photo enforcement case to be set for arraignment. A copy of your driver's license must accompany this form and be faxed to (303) 441-1811.

Waiver of Service and Request for Setting: Use this form to waive personal service of your photo enforcement violation and to request a court date.
