Trade Name Change Instructions

Pursuant to State Liquor Regulations, no licensee shall change the name or trade name of the licensed premise without submitting the proper documentation, not less than ten (10) days prior to the change of name.

Required Forms:
Permit Application and Report of Changes (DR8442)

Required Fees:
City of Boulder - $50.00
Colo. Dept. of Revenue - $50.00

Instructions:

PERMIT APPLICATION AND REPORTS OF CHANGES FORM

1)    You must fill in all of the blanks on the first page, all of this information can be found on your State liquor license.
2)    The applicant is the company or person that holds the liquor license, it is not the individual filling out the application.
3)    Check the "Change Corp or Trade Name Permit" box in Section C.
4)    Proceed to page 3 and fill out Question #6 in the "Change DBA or Corp Name" section.
5)    You must fill in all of the blanks in this section.
6)    Proceed to the "Oath of Applicant" section.  This must be read and signed by a representative of the applicant (i.e., corporate officer or the owner).  Please sign, title and date this section. 

Once you have completed the form, mail it along with the fees to:

Boulder, CO 80306

Do Not send the application to the State liquor division, the city will administratively approve the application and submit it to the State liquor division.  You will receive a certificate of trade name change from the State liquor division along with a new City of Boulder liquor license.  These should replace the existing licenses that you have.  All licenses must be in public view at all times.
